The Third Street Men's Shelter is a model program for 200 homeless men. In this nine-month program, residents work in stages toward health, sobriety, housing and jobs. This includes individual and group counseling, vocational training, assistance with entitlements and legal problems, and on-site medical and psychiatric services.We are currently seeking an Entitlements Coordinator to join our shelter team. Basic job functions include but are not limited to:
• Assessing clients’ needs for entitlement services and referring clients for Medicaid/Medicare, Public Assistance, & Food Stamps as needed
• Monitoring client compliance with referrals and reporting noncompliance regularly to each client’s assigned case manager
• Ensuring that all clients who are not employed on the books are receiving either Public Assistance or Social Security Benefits at all times
• Assisting case managers in helping their clients apply for Social Security Benefits
• Regularly updating the IMA computer system to reflect each client’s current entitlement status to ensure that case managers can access the most current information at all times quickly and efficiently
• Attending weekly case conferences with social service staff and providing entitlement case updates in order to ensure that each client smoothly & quickly transitions into housing
• Acting as the liaison with Federal, City, and State agencies providing entitlements services, including Social Security, Medicaid, HUD
• Providing education about Public Assistance & related programs, client documentation, immigration and other entitlements programs to clients and staff members
• Conducting monthly entitlement supervision with case managers, explaining HRA policies relating to attendance at job search programs, their metro card policy, the Fair Hearing and Sanction process, and providing clarification as needed on related issues
• Providing counseling around entitlements as needed
• Developing and maintaining database of referral resources
• Other duties as assigned
