PURPOSE: As part of a multi disciplinary team, this position advocates for, initiates and implements all aspects of a client's case plan concerning access to required resources, services and opportunities according to established agency standards.QUALIFICATIONS:
Bachelor's Degree in Social Work or related field or equivalent experience. Minimum of 1 year experience working with victims of domestic violence in a shelter setting. Demonstrated ability to develop and coordinate effective case plans. Have familiarity with community resources. Ability to perform community outreach. 40 hour domestic violence training required. Culturally sensitive and able to work with a multi cultural client/customer population; ability work in a non-judgmental manner and foster positive growth/outcomes; able to work as part of a team and maintain professional boundaries. Bi-lingual Spanish preferred.
RESPONSIBILITIES/ESSENTIAL FUNCTIONS:
The Case Manager will be responsible for managing case plans of shelter program clients, keeping statistical records, establishing client goals, monitoring of progress toward goals, assist with activities related to the case plan, meet with clients on a regular basis for case management, act as liaison to DPSS, GAIN, CalWORKs and attend related meetings or other meetings as assigned. Responsibilities include assistance with transitional and permanent housing. Responsible for provision of shelter coverage and assisting with all aspects of daily shelter service delivery. Assists shelter coordinator with daily operations. Answers crisis lines, performs intakes and monitors and enforces house routine, structure and safety. Performs crisis interventions and paraprofessional group counseling as assigned. Will be responsible for advocacy on behalf of clients to DPSS, courts, other related outside service referrals. Some holiday work required.
