Description
Will perform evaluations and develop service plans. Assist clients with goal planning and conduct regular reviews, provide support, assist in connecting clients with available community resources, and develop and maintain interagency relationships. Engage with potential clients upon their release from probation camp, leading to the provision of direct case management services. Identify existing services in the youth’s community and surrounding neighborhoods, provide referrals and monitor clients' participation in these academic enrichment and recreation activities. Maintain a regular case load in conjunction with other responsibilities that includes submission of accurate documentation of all services provided, including up-to-date case notes in the appropriate format. Develop individualized service plans in collaboration with parents/caregivers. Provide information and referral(s) to appropriate services in collaboration with probation officers working with the youth. Continually assess needs and provide advocacy support in obtaining appropriate services. Provide guidance and support to children and their families/caregivers in a variety of environments and community settings. Participate in team meetings.

Skills
AA degree plus 2 years experience in mental health OR BA/BS in social work, psychology or related plus 1 year experience in mental health. Strong knowledge of case management and mental health rehabilitation principles. Demonstrated ability to relate to culturally diverse clientele. Must be able to work as part of a team, as well as independently. Ability to exercise sound judgment in crisis situations. Effective verbal and written communication skills.

Notes
Able to work evenings and weekends when needed. Must have reliable transportation, valid CA driver’s license, proof of auto insurance liability insurance, and acceptable driving record. Bilingual in English/Spanish required.
