Description
Responsibility:
Under the supervision of the Program Manager, the Dual Diagnosis Case Manager is responsible:

•        Provide case management to clients that have dual or multiple diagnosis.
•        Refer target clientele to appropriate services (e.g. Edelman Westside Mental Health Center, Venice Family Clinic, Turning Point, Daybreak Shelter), per guidelines of the Santa Monica Dual diagnosis Program.
•        Develop effective, trusting relationships with clients that facilitate provision of case management services.
•        Co-facilitate engagement/retention and/or 12-step groups with Dual Diagnosis team member.
•        Provide case management and aftercare/follow-up services to clients placed in permanent housing through Dual Diagnosis and Shelter Plus Care programs.
•        Provide information, referral and social service advocacy as needed.
•        Maintain accurate records and statistical data (manually and via HMIS).
•        Provide case management training and on-going support to case managers.

•        Collaborate with Dual Diagnosis Program team members and outside agencies as necessary.
•        Participate in all case file, staff meetings, trainings and retreats.
•        Participate in on-going self evaluation, program evaluation, development and implementation.
•        Other Duties as assigned.

Qualifications/Requirements:
•        BA or equivalent work experience.
•        Demonstrated knowledge of and experience in dealing with issues faced by the population served (homelessness, mental illness, substance abuse, medical issues, and domestic violence).
•        Experienced in group facilitation.
•        Knowledge of social service agencies, especially on the Westside, including government benefits/Welfare system.
•        Skilled in non-violent crisis intervention.
•        Computer literate – able to use Client Track system for client file management.
•        Ability to self-motivate and work as part of a team.
•        Bi-lingual English-Spanish preferred.
•        Great organizational skills a must.
