JFS is seeking a social worker with older adult experience to provide comprehensive referral and case management services for frail elderly, Medi-Cal eligible clients and their families to allow clients to remain safely in the home setting.

Responsibilities include:

Receive program referrals and intakes

Provide telephone assessments of client referrals on a routine and daily basis

Disseminate program information accordingly

Conduct in-depth assessment covering psychological, social, physical, and environmental concerns

Develop a comprehensive care plan for assigned clients including all social and medical services needed by the client

Coordinate the delivery of all services in the care plan and follows up on client situation as needed

Review and authorize services every month

Offer crisis intervention and provide short-term or ongoing psychotherapeutic support to clients and families as needed

Identify and develop family and other informal support systems for the client

Refer clients to specialist consultants to provide additional detailed evaluation and recommendations for improvement of the client’s functional level

Case recording, maintenance of financial records and reporting on a timely and relevant basis;
Select, negotiate and budget supportive services for clients

Other related duties as assigned

Masters in social work or a related field; BBS registration preferred. Experience with care management for older adults in a community setting; superior verbal and written communication skills; computer literacy in MS Office. Demonstrated ability to work in an interdisciplinary community setting, exercise sound casework judgment and fiscal management skills;comply with documentation procedures. Bi-lingual English/Russian required; Spanish and/or Farsi fluency in addition a plus. EOE/AA
