The following position is currently available. All candidates must have a good driving record and a valid California Driver License.
On-call position in Five Acres’ RTC program. This person is responsible for working the treatment team to ensure the provision of case management services for severely disturbed children and adolescents in the residential treatment program at Five Acres in accordance with policies and the clients' treatment plans. This person will attend weekly cottage meetings and regularly meets with social workers to determine the treatment needs of each child for case management services; coordinates case management services to ensure the needed services are provided; schedules all Treatment Planning Review Meetings ensures that client visitation schedules are prepared on a weekly basis and routed to the appropriate treatment team members; makes necessary contacts with treatment team members (e.g., attorneys, CSW, family members, agency volunteers, CASA Workers, auxiliary treatment programs, school personnel, etc.); makes regular and timely phone contact to treatment team members regarding the notification of Significant Incident Reports; provides court ordered monitoring to visits between children and family member(s); transports children to other sites in order to facilitate family visits; provides feedback regarding the visitation to the unit clinician; ensuring that all contributing departments complete their assigned sections of the NSP by the deadline; assists in the regular scheduling of psychiatric consultation appointments for each child; initiates and tracks to completion the necessary documents to obtain Psychotropic Mediation Authorization; assists the clinician when hospitalizations occur in ensuring needed communications with family, etc. are completed and properly documented; documents all Mental Health services accurately, completely and in a timely manner; supports Agency philosophies, policies and practices; exercises good judgment in the performance and of duties and responsibilities; maintains confidentiality regarding children, families and operations of the agency. An undergraduate degree in social services, psychology or related mental health field and four years of experience working in a mental health setting; or AA degree with six years’ experience working in a mental health setting. Previous experience working with SED children and their families. Ability to pass utilize the Pro Act physical restraint system, Bilingual Spanish is preferred as well as the ability to work evenings and weekends. Flexibility and reliability is a must!
