Case Manager- Part Time

We are a private, small but growing care management and guardianship company, seeking an experienced human service professional to join our team. We serve individuals with disabilities throughout Cook County and surrounding areas. This would be an ideal position for someone looking to get back into the field after an absence or those searching for a long-term, part-time commitment. Full-time opportunities may be available based on future growth.

Responsibilities include:

>Visiting clients and monitoring their care while living at home or in a residential facility
>Identifying and securing or resolving issues with public benefits and health insurance
>Coordinating medical care, caregiver services, home health services, transportation, home modifications, medical equipment and any other service needed to allow individuals to remain in their home
>Transporting client to various locations within the community
>Attending IEP’s, care plan meetings, probate court hearing and medical appointments
>Arranging for recreational, social, educational and vocational programming as needed
>Interfacing with family members, attorneys, trust officers, medical professionals and other involved parties for the benefit of the client
>Shopping for client supplies including medication, clothing, household supplies and other necessary items
>Completing research on various topics as directed by the manager
>Writing reports on clients’ status using a company-issued template
>Inputting daily service notes into a computerized system

Skills
We are seeking a hard-working, detail-oriented individual with a minimum of a Bachelor’s Degree and at least two years experience working with elderly, medically, cognitively, physically, or mentally impaired populations. Professional presentation, great communication skills and the ability to work independently is a must.

Notes
The schedule for this position is two days per week from 9:00am-5:00pm (Times can be adjusted for the right person). We are hoping to find someone with flexibility as the exact days needed each week could fluctuate and we may end up adding an additional day as business increases. The pay rate is $150 per day. Benefits are not currently being offered for this position. Candidates must have a vehicle, auto insurance, cell phone, laptop computer and availability to work from home as needed.
