The Community Care Coordination program seeks a Clinical Coordinator to supervise a dynamic team of service providers. Candidates must be a licensed social worker (LMSW). Candidates must also be familiar with HIV and issues related to substance abuse, homelessness, chronic illness, mental illness, DV, child abuse, MICA clients, etc. The clinical coordinator will be primarily responsible for providing monthly supervision to case managers, monitoring staff fieldwork, reviewing progress notes for clinical content, reviewing and signing off and assessments and care plans and conducting monthly team meetings. Strong clinical, time management and writing skills (particularly with assessments) is a must.
