Monitor Clinical case coordinator

Monitor and provide oversight of recruiter/trainer

Encourage regular and consistent contact with birth, adoptive or foster family.

At admission, will be present or assign clinical case coordinator to be present to review medication and complete fire drill form.

Arrange and participate in disposition conference prior to completion of respite, if needed.

Review of home study prior to submission to OMH.

Education/Degree: LMSW
Years of Experience: 3 – 5 years, working with SED population, 2 years supervisory experience preferred
Valid NYS Drivers License
Team Player
Bilingual (Preferred)
