The Homebase program is seeking Case Managers to work with families at risk of homelessness.

Responsibilities include providing direct services to clients experiencing or at-risk of homelessness, such as conducting intakes, engaging clients, providing assessments, developing service plans, providing referrals, client advocacy, counseling, follow-up services. Case Managers are responsible for a full case load, often with individuals in crisis. They will conduct home visits and other field work, monitor and document client progress toward service plan goals, and document all interactions with, or on behalf of, clients. Case Managers will maintain familiarity with program resources available on- and off-site and coordinate client services with other social service, mental health, medical, employment, educational, child care and other provider, as well as attending Housing Court hearings as needed.

A Bachelors degree is required, with a minimum of one year experience preferred. Strong oral and written communication skills are required. Bilingual (English/Spanish) abilities strongly preferred.
