Westchester Jewish Community Services (WJCS) is a large, not for profit, organization with headquarters in White Plains, NY. We are currently searching for a full-time, 35 hours per week, Service Coordinator to work in White Plains, NY.
The Service Coordinator assesses, develops treatment/service coordination plan, implements the plans, follows-up and advocates for individuals with special needs served within the Community Residential Care (CRC) division of WJCS. Other job duties of this position include: Implementation of the preliminary Individualized Service Plan as well as the Individual Service Plans; the review of treatment plans at least semi-annually. The Service Coordinator also initiates and maintains individual records and completes all documentation necessary to reflect all case management service provision. Qualifications include: A minimum education level of a bachelors degree in a health or human service field is necessary. Registered Nurses can apply for this position as well. One-Year experience working with people with a developmental disability or working as a service coordinator with any population, unless you hold a Master's Degree in a health or Human Services field. A Valid New York State license in addition the ability to travel from one site to another is a must. Computer literate.
